About Questionnaires
- UserZoom studies offer a variety of questionnaires to customize your study.
- Most are not mandatory and allow you to decide if the questionnaire fits your study.
- Questionnaires added with no questions are not shown to participants.
- Questionnaire types available depend on the study type and task type.
- Surveys consist of a single questionnaire task.
Study Level Questionnaires
- Study level questionnaires include:
- By default, they are in these study types:
- Basic Usability Test & Basic Usability for apps
- Live Intercept & Live Intercept for apps (The flow of Live Intercept studies depends heavily on if the Initial and/or Final Questionnaire contains questions.)
- For study types without these as default, add a Questions only task to serve the same purpose.
Initial Questionnaires
- Study Type Availability: Unmoderated/Basic Usability Tests and Live Intercepts
- The Initial Questionnaire is often used for socio-demographic questions, brand awareness, interests, needs, and habits of conduct.
Final Questionnaires
- Study Type Availability: Unmoderated/Basic Usability Tests and Live Intercepts
- The Final Questionnaire is often used to gather general evaluation and feedback of the studied website(s) or app(s).

All Other Study types

Questionnaires associated with a task are on the Study Builder tab under the task to which they belong.

Using questionnaires included in a task (as opposed to preceding or following Questions only tasks) is helpful when you want to randomize. No matter the randomization, these questionnaires remain attached to the task.

Pre-Task Questionnaires
- Pre-Task Questionnaires ask questions related to the upcoming task.
- Participants answer questions before they see the actual task.

Pre-Task Questionnaires are not available for these task types:
- Navigation Only Tasks
- Questions Only Tasks
General Questionnaires
- The General Questionnaire comes after any other questionnaires in a task and is used to ask questions related to the task participants just completed.
- All participants see this questionnaire no matter their success in the task.
- General Questionnaire is not available for these task types:
- Navigation Only Tasks
- Questions Only Tasks
Intercept Questionnaires
- Intercept Questionnaires are exclusive to Navigation tasks in Advanced UX Research studies and allow you to intercept participants during a Navigation Task (if they visit specified URLs) to ask them questions.
- After answering questions in the Intercept Questionnaire, participants continue from where they left off in the Navigation Task.

- Intercept Questionnaires are not available for Advanced UX Research for Apps.
Validation Questionnaires
- Validation Questionnaires are exclusive to Navigation tasks in Advanced UX Research studies.
- Use this questionnaire to evaluate if a participant was successful in the Navigation task.

Success Questionnaires
- Success Questionnaires are exclusive to Navigation tasks in Advanced UX Research studies and are shown to participants who successfully complete a Navigation task.

Use Success Questionnaires to:
- Ask more questions about the interaction
- Rate specific aspects in the task
Non-Success Questionnaires
- Based on Navigation task settings in an Advanced UX Research study, participants who do not complete the task successfully see a Non-Success Questionnaire.
- There are three questionnaire types: Abandon, Error, and Timeout.
Abandon Questionnaire
The Abandon Questionnaire is shown to participants who click the Abandon button and allows you to ask why participants gave up on a task or if they had issues.
Error Questionnaire
- The Error Questionnaire is shown to participants who click the Success button, but did not:
- Reach the correct URL (Validation by URL)
- Select the correct validation question (Validation by Question).
- This questionnaire allows you to find why participants thought they were successful even though they were not.
Timeout Questionnaire
- The Timeout Questionnaire is shown to participants who spend more time than expected to complete a Navigation task (more time than the defined timeout).
- This questionnaire allows you to ask participants why the task was difficult for them.
